“Forty-nine,” Izuku growled. He fumbled around, trying to find the zip to Kacchan’s suit before tearing into it, like it’d somehow personally offended him. He slung a naked Kacchan over his shoulder and carried him into the bedroom, ignoring Katsuki’s protests about their food. He’ll put it away with Black Whip once he gets Katsuki on his cock.

Stuck in a ghost-like state, the boy enters a race against time in order to save himself from permanently dying. Much to his luck - or lack of it -, the only person who can see and talk to him in this state is no one other than Kacchan.
Alternatively: Deku and Kacchan are soulbound.
