Dr. Otther is vampire dentist and his dentistry chain is both his last name and play on words. Because he provides dentisty for everything 'other' than humans. And he hires medical massage therapists to come and treat himself and the other dentists and dental hygenists because they all basically hunch over to work on everyone's mouths and need these treatments for the toll that it takes on them and himself. And the joke is, because he is so old, that his knots in his shoulders have been there for at least a century or three.
