The Doctor is supposed to regenerate, but because he put his regeneration energy through the heart of the Tardis, the Tardis used it to pull the last person who looked into the heart of the Tardis to her, only she did not mean to swap her for the Doctor.
The Doctor appears in Pete’s world and is reunited with Rogue and the Metacrisis Doctor, but this world is all wrong, or, actually, the world the Doctor had come from was all wrong, because why else would both he and Rogue remember a time that Mavity was called Gravity?

“Baba,” Benji whimpers. Sybelle says nothing at all.
“I’m here.” He cards through Benji’s curls, tucking his chin over the boy’s head. “I’m here. Whatever happens, I’m here.”
The carriage rattles. He’s fairly sure they’re in some kind of shipping container, if the corrugated walls are anything to go by. Perhaps on the back of a truck, or loaded onto a train.
It does not sway like a ship. This, at least, is a comfort.
