Summary
Akutagawa was an independent worker. he had never chosen to work with a team, and he was very confidant that given the choice, the Armed Detective Agency wouldn't have worked with him either. They didn't have any sort of obligation to him, just like he had none to them. That didn't mean it doesn't sting when they ignore his injuries in favor of someone else's though, just because they were a bit worse.
OR
The five times Akutagawa's injuries were ignored because someone else's were worse, and the one time someone realized just how bad things were.

Ryuunosuke is quiet for a second while he thinks of a response and fights down a blush. He can’t say why he got distracted; he can’t just tell his friend ‘Oh, I’m sorry, I was just surprised at how firm your core muscles are when your belly looks so soft.’ He wouldn’t ever live that down.
