In the battle against Kaguya, Naruto's Harem no Jutsu fails spectacularly.
All hope seems to be lost.
Until Sakura meets the Sage of Six Path, who gives her the chance to go back in time and fix everything.
He just didn't mention that the body she would end up in would be male, and, more to the point, already meant to have an occupant.
It is only through dumb luck that she didn't kill her most precious soul-brother.
She will hold a grudge for that. She will.
But in the meantime she has brand new siblings (she always wanted siblings) to see raised well, safe and happy.
Oh, and also a world to save.
But that will keep.Or: Sakurama becomes the Inner/in-built twin to one of the -ama brothers. She has always been intense and possessive, and at least her new siblings welcome that.

"Maa, did I do you some kind of favor without noticing, kunoichi-san?" He finally asked, intrigued despite himself.
She took a pen out of her forearm bracer and twirled it between her fingers with a predatory smile. "Oh, no. I just thought I'd prepare it in advance so you only have to sign it."
"Oh, and why-ever for?"
She flipped the pen over so that he could easily take it from her hand, 'coincidentally' pointing it straight at his nose in the process. She shot a quick sideways glance at Sakura before her eyes came back to his and smiled in a way that scrunched her nose cutely. "You sign it, and I take care of your... problem for you." She answered him with a tasteful eyebrow waggle. Kakashi had no idea how she managed to make eyebrow waggle tasteful, seduction specialist sure were something else.
Or: Kakashi is despairing about Sakura's habit to wear civilian perfume, and well on his way to becoming canon Kakashi. Except a chunin kunoichi sees it and decides it's an excellent Opportunity to get Kakashi to owe her one

Summary
The Fourth Shinobi War wasn't going well. Shinobi were dying right-left-and-center. Ono Toshiko had done what she could to avoid it - and failed.
Her little brother had a plan. A fail-safe. A seal attached to her heart, mind, and chakra network so that if his latest knuckle-headed scheme to fix their world failed, there would still be hope.
She agreed but prayed nonetheless that a miracle would occur and after the war was over Naruto would take the seal off of her.
She should have known better.
After all - if there were deities listening, they clearly had no mercy for a reincarnated soul. Especially not enough to *not* force her to do it all over again. Only in a new body, and a new time.
