When quirks manifested all those years ago, so did the appearance of soulmates. As such, only those with the quirk gene had a soulmate. There was only one difference between a normal person and your soulmate: You can't lie to them. Katsuki realises Deku is his soulmate extremely young, but Izuku remains oblivious. Under the impression that he doesn't have a soulmate, Izuku would never suspect that Kacchan was his soulmate.
