The burning pain is starting to come back again. It begins in his chest this time, slightly to the right. Yuuji swallows, reaching up to press his hand against it.
So, he thinks dazedly, as the universe begins to take him apart. This is what happens when you break a Binding Vow.

Megumi dies in Shinjuku. Yuuji is thrown back in time to save him. Again, and again, and again.

A blink, a slow turning of the head. Midas-spun sun, sinking its long gold teeth into Yuuji’s frame. Against the light, he’s darkening in ink blue. “Yeah? What’s up?
“Let’s go home.” Megumi murmurs.
Yuuji's eyes soften. “Okay.” He says, and pulls Megumi close. A sweet kiss presses itself into the seam of his mouth, thick with summer.
Or: Over the course of a single summer, slipping through their fingers like water, Yuuji and Megumi learn the intricacies of each other.

“There’s no easy way to say this, Itadori. The cancer is back and it’s spread to your stomach.”
The corners of his lips twitched, smile faltering for merely a second. He felt his palms begin to sweat as he kneaded and gripped at the fabric of his shorts, faded by the sun and beginning to wear thin. His vision began to fade at the edges, darkening while everything else began to brighten. The distant sounds of the hospital faded outside the examination room they were in, sounding as though he was underwater. Drowning.
“Did you hear me, Itadori?”
Doctor Shoko’s voice came, slicing through. Despite her usual vacant expression being ever present, he could see the crease of her eyebrows, concerned. She was worried. He didn’t want her to worry.
Letting out a hesitant and somewhat awkward laugh, he reached up, smiling, rubbing the back of his neck. It felt cold.
“Hah, jeez! That wasn’t what I wanted to hear!”

A.k.a. Yuuji Itadori is dying of cancer, Fushiguro Megumi is grappling with his sister's decade-long coma, and neither of them feel deserving of love. On top of that, they have homework due.

Summary
After a chance encounter with a Knight named Fushiguro Megumi in the forest, Itadori Yuuji assumes their paths will never cross again. But when her village is destroyed by Curses and burned to the ground, Yuuji enters the Gojō Academy of Knights to honour her grandfather’s last wish.
There, she faces the brutal reality of slaying Curses, a reality that slowly erodes her innocence. Meanwhile, unseen forces, both friend and foe, quietly push her toward a fate she does not yet understand. Everything culminates when a mission under her leadership goes horribly wrong.
Given no time to grieve, Yuuji is whisked away to the royal palace and told she is the lost princess of the Kamo Kingdom. An attempt on her life follows soon after, and Megumi unexpectedly returns as her newly appointed bodyguard.
As danger closes in, the two must navigate a hesitant friendship strained by their new roles, while the threat surrounding Yuuji’s bloodline begins to surface—such as the existence of an uncle tied to an enemy kingdom, whose identity everyone are reluctant to reveal to her.
