Summary
After being captured by All for One and given to the Doctor, there is very little that Izuku can trust anymore. Not his body, not his loved ones, and certainly not his mind. There isn't a single aspect of himself that hasn't been violated and altered. He does his best not to let them break him, but there are some things that even he can't smile through. Eventually, he doesn't have the awareness to care anymore.
When he is finally rescued, he's barely a shell of his former self. There's no laughter, no mumbling, no conviction, only blank stares and feeble responses. Despite what they are told, his classmates are determined to bring him back.
“Hello all,” Mrs. Midoriya managed a small wave. “It’s nice to see you here, but I thought Izuku couldn’t have visitors other than family come through this door.”
“He can’t.” Todoroki confirmed with a nod from the others. “That’s why we entered through a window instead.”

But he trusts Todoroki well enough (and the sound of a hug, right now, of warmth and a heartbeat and breathing, so different to mere hours ago, sounds beyond wonderful-) and so he doesn't really have to think before he nods a little, messy but there all the same.
