The Grand Line Therapeutic Centre for Distressed Alphas is a state-of-the-line medical facility specializing in treating Alphas. Their Feral Alpha Department is its shining jewel: after all, when Alphas experience immense trauma and lose themselves to their instincts, not a lot can bring them back.
Sanji is an Omega nurse in that department. Vivi is good with terrified Alphas who can't make sense of their new bearings; Nami is great with Alphas that are mostly bark; and Sanji is known for handling even the most aggressive Alphas that charge him head-on.
When a grassy-haired, mud-covered feral Alpha takes four first respondents to subdue and tranquilize him, before being admitted to the centre, the job has Sanji’s name all over it.
In that sturdy, private patient room, they fall in love.

Zoro’s ears and the back of his neck turn red. “When I talk to you, I expect prompt, clearly spoken, polite responses, not all this… barking.” He makes a vague gesture at Zoro. “Until I get that, however, I will be treating you how your behavior tells me you wish to be treated.”
Zoro crosses his arms and turns to face Mihawk again, face flushing when Mihawk just nods at his feet like he expects Zoro to sit there.
